Aetna (except for CVS plans)
Blue Cross Blue Shield of Arizona (most plans)

UnitedHealthcare (UHC/UBH)
Aetna CVS plans, Medicare, and Medicaid (including AHCCCS) are not currently accepted.
Because coverage varies by plan, you may need to verify your benefits directly with your insurance carrier.

Out-of-Network & Private Pay
If I am not in-network with your provider, I offer private pay options. I can provide a superbill (detailed session receipt) for you to submit to your insurance for possible out-of-network reimbursement.
